Abstract
Pulmonary tuberculosis is an infectious disease caused by Mycobacterium tuberculosis. TB is still a global health problem. Approximately one third of the world population is infected by Mycobacterium tuberculosis and the source of infection came from smear positive and negative patient. Smear negative pulmonary TB can be considered based on clinical symptom and chest x-ray as well as description of TB and it’s depend on doctor's decision, so this can lead to under or over-diagnosis. Molecular examination with real time PCR method of genexpert is a breakthrough invention to diagnose TB quickly. The limitation of diagnostic methods is one of the obstacles in increasing the findings of pulmonary TB cases. The diagnosis of pulmonary tuberculosis (TB) is important in reducing morbidity and mortality. Laboratory diagnosis of pulmonary TB is based on smear examination and tuberculosis culture has low sensitivity. The main challenge in diagnosing TB conventionally is the lower detection sensitivity value on microscopic examination compared to culture and the length of time required for culture. Methods. Design of this study uses a diagnostic test with a cross sectional design. Population are all patients who have BTA Sputum examination at Laboratory of Prof Dr WZ Johanes Kupang Hospital on October 2019. Result. Diagnostic value of genexpert for diagnosing smear negative pulmonary tuberculosis are sensitivity 100%, specificity 97,06%, positive predictive value 97,92%, negative predictive value 100% . Coclusion. Genexpert has a high sensitivity, specificity, positive predictive value, negative predictive value in negative smear pulmonary TB in establishing a diagnosis of tuberculosis in suspected pulmonary TB compared with smear sputum examination.
